A care plan is a patient-centered document created between the patient and their provider to meet the goals and needs of the patient’s health status. It may include an array of treatments, lab orders, education advice, or referrals to address one or more multiple conditions. A care plan can provide guidance for the patient in order to reach the best healthy outcomes.
